• McKinney-Vento Form

  • McKinney-Vento Form English

  • McKinney-Vento Form Spanish

  • McKinney-Vento Form Marshallese

• Pre-Emergency Evacuation Form

• Free Breakfast and Lunch Documentation Fee Waiver (complete only if applicable)

Bring the completed documents (above) to the main office, along with:

• Original birth certificate of the student

• Original immunization record

• Guardian photo I.D.

• Two proofs of residency

At West Bountiful Elementary, we consider each child's academic, social, and emotional needs gathered from teachers, administrators, and parent insights as we design class lists. Significant time and effort goes into creating balanced classes for ~700 students. If you have an educational concern for your child, please complete and submit this form (link above). We take all information into consideration as we place students in the best educational environment to meet their needs.
